Going Away With Scheme

Request to county to appoint a mentor or assessor

This form should be completed by the commissioner who is responsible for approving the Residential Event Notification Form; this could be a district, division or the county commissioner. A commissioner cannot approve an event she is attending.

We ask that this form is completed a 6 months before the intended event to allow adequate time to work through the Going Away With Scheme and for us to find a mentor and assessor.
